Summary
Oregon Health Authority is modifying the Care Coordination reporting requirement for CCOs under CY 2025 contracts. The reporting period start date for the June 30, 2025 report will be adjusted to provide at least 90 days from when OHA provides the reporting template to CCOs. CCOs may optionally report on activities prior to the adjusted start date, but are not required to do so. The due date remains August 14, 2025.
