Summary
Oregon Health Plan (Medicaid) is extending the due date for the 2025 Patient-Centered Primary Care Home (PCPCH) Annual Report from TBD to January 5, 2026. Coordinated Care Organizations (CCOs) must submit facility-level data about all Members assigned to PCPCH Providers in CY 2024 using the 'PCPCH Form' survey in the CCO deliverables portal.

By January 5, 2026: CCO administrative staff (not clinical billing teams) must ensure the annual PCPCH report with facility-level member assignment data is completed and submitted via the 'PCPCH Form' survey in the CCO deliverables portal. A reference copy of the survey is available on the CCO Contract Forms webpage. Direct questions to Cheryl Henning at Cheryl.L.Henning@oha.oregon.gov. This is an administrative reporting requirement for CCOs, not a direct clinical billing workflow change for provider practices.
