Action Required
By January 30, 2026: Billing/compliance team should monitor for OHA guidance documents on FWA Prevention Plan and Assessment Report requirements. By April 30, 2026: Ensure your organization (if a CCO) or contracted CCO partner submits the CY 2026 Annual FWA Prevention Plan and CY 2025 Assessment Report to OHA. Verify that compliance and quality assurance teams are aware of the extended deadline. Contact Cheryl Henning (Cheryl.L.Henning@oha.oregon.gov) or HSD.QualityAssurance@odhsoha.oregon.gov with questions. Failure to submit by April 30, 2026 may result in contract compliance violations.
