Summary
Aetna has updated its viscosupplementation (hyaluronate injection) policy for knee osteoarthritis, effective immediately. The policy requires precertification for all viscosupplementation products, establishes strict medical necessity criteria including failed conservative and pharmacologic treatments, and clarifies that combination therapies and off-knee indications are not covered. Billing teams must implement precertification workflows and enforce the detailed approval criteria to avoid claim denials.
Action Required
IMMEDIATELY: (1) Billing and precertification teams must implement mandatory precertification requirement for ALL viscosupplementation products before claim submission. Contact Aetna precertification at (866) 752-7021 or fax (888) 267-3277. (2) Update billing software to flag CPT codes 20610, 20611, and related arthrocentesis codes when billed with viscosupplementation diagnoses to require prior authorization. (3) Providers must document ALL medical necessity criteria in patient records before scheduling: radiographic OA evidence, failed non-pharmacologic treatment (physical therapy, exercise, bracing, weight reduction), failed 3-month trial of analgesics (acetaminophen, NSAIDs, or topical capsaicin), AND failed 3-month trial of intraarticular steroid injections. (4) Update encounter forms and EMR templates to require providers to confirm: knee pain interferes with functional activities, patient is NOT scheduled for total knee replacement within 6 months, and all prerequisite treatments have been attempted. (5) Educate front desk and prior auth staff that CPT 27369, 73580, and 77002 are NOT covered for viscosupplementation guidance purposes. (6) For continuation therapy: verify patient experienced improvement in pain/function AND at least 6 months elapsed since last injection series before approving. Failure to obtain precertification or meet criteria will result in automatic claim denials.
