Summary
Aetna's videostroboscopy policy (CPB 0305) clarifies that CPT 31579 is covered only when used as a diagnostic procedure for vocal cord pathology detection in patients who have already undergone mirror-image and endoscopic examinations with no abnormal findings but have persistent symptoms. Artificial intelligence-based assessment of videostroboscopy for vocal fold sulcus diagnosis is considered experimental and not covered. All other indications remain experimental/investigational.
Action Required
By April 17, 2026: Billing team must implement prior authorization review process for CPT 31579 (laryngoscopy with stroboscopy). Require documentation in claims showing: (1) prior mirror-image examination performed, (2) prior endoscopic examination performed, (3) no abnormal findings on both prior exams, and (4) persistent symptoms justifying videostroboscopy. Update billing software to flag claims for CPT 31579 without this documentation for denial. Deny all claims for CPT 31579 when used for indications other than the approved diagnostic scenario. Explicitly deny any claims referencing artificial intelligence-based assessment of videostroboscopy for vocal fold sulcus diagnosis. Train providers and billing staff that videostroboscopy is only covered when preceded by negative mirror-image and endoscopic examinations. Claims submitted without meeting these criteria will be denied.
