Summary
Priority Health requires all contracted providers in Medicare and Medicaid networks to complete annual Model of Care (MOC) training for HIDE-SNP and D-SNP plans by December 31, 2026. This is a new CMS requirement, and training must be completed and attested through either individual online registration or bulk roster submission. Failure to complete by the deadline will result in incomplete provider status in the system.
Action Required
By December 31, 2026: All providers contracted with Priority Health in Medicare or Medicaid networks (including specialists, ancillary providers, and ICT members) must complete the 2026 HIDE-SNP & D-SNP Model of Care training. Billing and practice management staff should: (1) Identify all affected providers in your practice; (2) Ensure providers complete training via Option #1 (individual on-demand webinar registration) or coordinate bulk attestation via Option #2 (roster submission to DSNPtraining@priorityhealth.com); (3) Verify correct provider NPIs are submitted—incorrect NPIs will result in "incomplete" status; (4) Document completion for compliance records. Out-of-network providers seeing 5+ HIDE-SNP or D-SNP members are also required to complete training. Late submissions will not be accepted and incomplete status may affect provider eligibility and claims processing for these plan types.
