Vesicoureteral Reflux Treatment by Endoscopic Injection of Bulking Agents (CPB 0534, reviewed 2026-07-30)

By July 30, 2026: Billing team must implement the following: (1) Update prior authorization software to require approval for CPT 52327 and related codes only when patient meets one of the eight specified medical necessity criteria (failed conservative treatment AND one of: previous failed ureteral re-implantation, medication intolerance, thick-walled neuropathic bladder, renal parameter deterioration, grades I-III reflux, persistent post-pubertal female reflux, recurrent febrile UTIs, or symptomatic post-transplant VUR); (2) Deny authorization and mark as experimental/investigational for claims using L8603, Q2026, or injections of autologous blood, calcium hydroxyapatite, chondrocytes, Durasphere, fat, Contigen, Zyplast, myoblasts, Bulkamid, or Vantris; (3) Configure system to deny coverage for any treatment sessions beyond the third injection for the same patient/diagnosis; (4) Verify patient has ICD-10 code N13.70-N13.9 with documented failed conservative therapy; (5) Add clinical note requirement to encounter forms requiring provider documentation of which specific criterion is met. Failure to implement will result in incorrect claim denials or inappropriate approvals for non-covered treatments.
