(2) DENY all claims that list scoliosis as the sole or primary indication (M41 codes without thoracic insufficiency context), congenital kyphosis (Q76.411-Q76.419), Poland syndrome, or hyper-kyphosis (M40 codes). (3) Educate providers that VEPTR is experimental for early-onset scoliosis, idiopathic infantile scoliosis, and kyphoscoliosis without documented thoracic insufficiency. (4) Add coverage criteria checklist to encounter forms requiring confirmation of patient age (6 months to skeletal maturity), presence of proximal ribs, adequate soft tissue coverage, and adequate bone strength. Failure to apply these restrictions will result in claim denials and potential overpayment recoupment.
