Summary
Aetna updated its Clinical Policy Bulletin for Testosterone Cypionate Injections (CPB 1014) effective July 13, 2026, establishing clear coverage criteria for two primary indications: primary/hypogonadotropic hypogonadism (requiring two confirmed low morning testosterone levels) and gender dysphoria (with specific requirements for informed consent, comorbidity control, and fertility preservation counseling). All other indications, including age-related hypogonadism and female sexual dysfunction, are classified as experimental/investigational and not covered.
Action Required
By July 13, 2026: Billing team must implement prior authorization requirements for testosterone cypionate injections (J1071, J1072) in the billing system and EMR. Require providers to submit documentation of (1) two confirmed low morning testosterone levels for hypogonadism cases, OR (2) gender dysphoria diagnosis with informed consent, comorbidity control, fertility preservation counseling, and specialized provider involvement (pediatric endocrinologist, family/internal medicine physician, or ob-gyn with mental health collaboration for patients under 18). Update encounter templates to prompt for these required elements. Additionally, configure system to deny claims with ICD-10 codes F52.0, F52.22, I50.1-I50.9, N95.1, R41.81, Z78.0, and Z85.46 (experimental indications). For gender dysphoria in adolescents, verify Tanner stage 2+ documentation. Claims submitted without required documentation or with non-covered diagnoses will be denied. Communicate these changes to all providers prescribing testosterone cypionate and front desk staff handling prior authorization requests.
