Summary
Aetna updated its medical policy on non-invasive negative pressure ventilation (body ventilators and poncho wrap) effective April 16, 2026. The policy clarifies that these devices are medically necessary DME for patients with stable/slowly progressive respiratory failure due to neuromuscular diseases, chest wall deformity, or central hypoventilation syndromes, and allows coverage of a second ventilator if it serves a different clinical purpose. All other indications (including acute hypoxemic respiratory failure) remain experimental/unproven and non-covered.
Action Required
By April 16, 2026: Billing team must update claim submission protocols for HCPCS codes E0457 (chest shell/cuirass) and E0459 (chest wrap) to require documentation of patient diagnosis matching covered indications (stable/slowly progressive neuromuscular disease, chest wall deformity, or central hypoventilation). Claims with ICD-10 codes J96.00-J96.02 (acute respiratory failure) will be denied; only J96.10-J96.12 (chronic respiratory failure) are covered. Update billing system to flag claims requiring prior authorization verification and ensure providers document medical necessity including PaCO2 levels (>45 mm Hg) or nocturnal oxygen desaturation data. For dual-ventilator requests, require clinical justification showing different purposes (e.g., wheelchair-mounted vs. bed-use). Failure to obtain proper documentation will result in claim denials from Aetna.
