Summary
Aetna has classified intra-oral appliances (such as NTI-tss devices) as experimental, investigational, or unproven for headache and trigeminal neuralgia treatment, effective immediately. Billing teams must deny or reject all claims for these devices when billed for headache or trigeminal neuralgia indications; however, coverage may apply for TMJ syndrome under separate policy CPB 0028.
Action Required
Immediately effective 2025-12-29: Billing team must update claim processing rules to deny or reject all Aetna claims for CPT 21110, HCPCS codes D7880, D8210, D9940, D9947, D9948, D9949, D9953, D9954, D9955, E0486, and K1027 when linked to ICD-10 codes for migraines (G43.001-G43.919, G43.E01-G43.E19), other headache syndromes (G44.001-G44.89), trigeminal neuralgia (G50.0), post-lumbar puncture headache (G97.1), or unspecified headache (R51.0-R51.9). Update billing system edits to flag and block submission of these code combinations. Providers must be notified that intra-oral appliances are no longer covered for these indications by Aetna; appliances for TMJ syndrome remain covered under CPB 0028. Front desk and authorization staff should route patient inquiries regarding these devices to providers for alternative treatment discussion. Failure to implement these edits will result in claim denials and potential compliance issues.
