Aetna CPB 0572 (updated 2025-08-26) establishes that home/ambulatory spirometry is medically necessary ONLY for lung transplant recipients (T86.810-T86.819, Z94.2), but is considered experimental/investigational for all other indications including asthma, cystic fibrosis, COPD, and other chronic pulmonary diseases. Billing teams must restrict coverage to transplant-only scenarios and deny claims for non-transplant indications.

Effective immediately (2025-08-26): Billing team must configure system to ALLOW coverage for CPT codes 94014, 94015, 94016 and HCPCS E0487 ONLY when ICD-10 diagnosis is T86.810-T86.819 (lung transplant complications) or Z94.2 (lung transplant status). For all OTHER diagnoses (asthma E84.0-E84.9, COPD J40-J94.9, IPF, ILD, cystic fibrosis, emphysema), configure system to DENY claims and append reason code: "Experimental/investigational - not covered under this policy." Update claim adjudication rules in billing software immediately. Train billing staff to flag any spirometry claims not associated with lung transplant diagnosis codes for manual review and denial. Implement claims blocking logic to prevent accidental approval of non-transplant spirometry claims.
