For HMO plans: deny coverage for bathtub safety frames, bathtub lifts, bathtub transfer boards, bed baths, raised toilet seats, bathing systems/shower chairs, and toilet safety frames—approve only commodes and bed pans for bed/room-confined members. For traditional plans: approve these items when medical necessity criteria are met. Update prior authorization templates to require documentation of: (1) specific medical condition requiring the equipment, (2) face-to-face or telehealth exam within 6 months, (3) for bathing systems: occupational/physical therapy evaluation and home assessment by Assistive Technology Professional, and (4) trial documentation showing lower-cost alternatives won't meet needs. Ensure claims flagged as duplicate/second equipment are denied as convenience items. Providers should document home confinement status, inability to access toilets, or specific mobility limitations on all DME orders. Failure to apply correct plan-specific rules will result in inappropriate denials or overpayments.
