Summary

By December 11, 2025: Billing team must update authorization and claim validation rules in the billing system to enforce the following: (1) CPT 38232 and 38241 are COVERED ONLY for autologous HCT in relapsed germ cell tumors, consolidation therapy for germ cell tumors in complete remission, or tandem autologous HCT for relapsed germ cell tumors—verify ICD-10 diagnosis codes C56.1-C56.9 or C57.4 with appropriate clinical documentation before processing; (2) CPT 38205 and 38240 (allogeneic HCT) and any HCT for epithelial ovarian cancers (ICD-10 codes not specified for epithelial type) must be flagged as experimental/investigational and require prior authorization denial or referral to clinical review; (3) HCPCS S2150 follows the same coverage rules as CPT codes—deny or require prior auth if criteria not met. Providers must submit clinical documentation specifying tumor histology (germ cell vs. epithelial) and treatment status (initial vs. relapse vs. consolidation). Front desk and prior auth staff should cross-reference diagnosis codes against tumor type before authorization. Claims submitted without proper clinical documentation or for ineligible scenarios will be denied.
