Summary
IHCP (Indiana Medicaid) requires transportation providers to use modifier XE instead of modifier 76 when billing multiple trips for the same member on a single date of service. MCEs must reprocess denied claims from January 1, 2025 forward by October 19, 2025.

Immediately: Transportation providers must stop using modifier 76 and switch to modifier XE for multiple trips on same date of service. By October 19, 2025: MCEs must reprocess all denied claims with XE modifier from January 1, 2025 forward. Within 90 days from August 19, 2025: Providers can resubmit previously denied claims using XE modifier (include first page of bulletin for timely filing). Update billing software to use XE modifier for additional transportation trips instead of modifier 76.
