Summary
Aetna's updated Clinical Policy Bulletin 0708 clarifies coverage for metatarsal phalangeal joint replacement procedures. Only hemiarthroplasty and total prosthetic replacement with silastic implants (In2Bones RTS and Primus Flexible Great Toe) are medically necessary for hallux rigidus; all other implant types including ceramic, modular, bioabsorbable, and metallic prostheses are classified as experimental/investigational and not covered. Billing teams must ensure claims for non-covered implants are denied and provider documentation reflects only approved implant types.
Action Required
Immediately implement the following: (1) Billing team must update claim processing system to DENY any claims for CPT 28291 paired with experimental implants (Accu-Joint Hemi, RegJoint, Moje ceramic, InterPhlex, Arthrex modular, Cartiva, METIS, OsteoMed ReFlexion, ToeFit-Plus, or Roto-Glide). (2) Update claim adjudication rules to ALLOW only L8641/L8642 when paired with In2Bones RTS or Primus Flexible Great Toe implants; deny all other implant types. (3) Create a preclaim validation checklist for CPT 28291 requiring: documentation of implant type in operative notes, verification that implant is on approved list (In2Bones RTS or Primus only), and ICD-10 diagnosis limited to M20.20-M20.22 (hallux rigidus only—NOT M20.10-M20.12 hallux valgus or M87.071-M87.079 avascular necrosis). (4) Update provider superbills and encounter forms to list only covered implants. (5) Brief all front-desk and authorization staff: any request for non-listed implants must be pre-approved as experimental or denied. Consequence of inaction: Claims for non-covered implant types will be rejected, delaying provider reimbursement and requiring claim resubmission or patient balance billing.
