Summary
IHCP has corrected a system issue that was incorrectly denying claims for procedure code 93298 (cardiac rhythm monitor evaluation) when billed with modifier 26 (professional component). Claims will be automatically adjusted or reprocessed starting October 15, 2025.

No immediate action required. Billing team should monitor remittance advices beginning October 15, 2025 for automatically adjusted or reprocessed claims with ICNs beginning with 52 or 80. Review Professional Fee Schedule updates on IHCP website. If you have previously denied claims for 93298 with modifier 26 from August 28, 2023 forward, these should be automatically corrected.
