Summary

Aetna has updated its rosacea coverage policy (CPB 0547) clarifying that medical treatment is covered, but surgical/cosmetic treatment of disfigurement is not. Topical oxymetazoline (Rhofade) is now explicitly covered for persistent facial erythema. A comprehensive list of experimental/investigational treatments and biomarker tests are NOT covered, including botulinum toxin, photodynamic therapy, and various serum biomarker measurements.

(2) HCPCS codes J0585-J0589 (botulinum toxin) are NOT COVERED for rosacea—add system block to deny claims with ICD-10 L71.x paired with these codes. (3) CPT codes 15780-15793, 17000-17111 (dermabrasion, chemical peels, laser destruction) are NOT COVERED for rosacea cosmetic indications—require ICD-10 code review before processing. (4) CPT 0232T (platelet-rich plasma) is NOT COVERED—auto-deny with explanation. (5) CPT 96931-96936 (reflectance confocal microscopy) and lab codes 82247, 82248, 84550 are NOT COVERED—add to experimental/investigational denial list. (6) Topical oxymetazoline (Rhofade) coverage is NEW—ensure claims are NOT denied when billed with L53.9 (persistent facial erythema from rosacea). Update prior authorization request forms to clarify cosmetic vs. medical-necessary surgical procedures. Train billing staff and providers that only CPT 10040 and 30120 require medical necessity documentation for surgical treatment.
