(2) Add eligibility verification checklist to ensure patients meet ALL criteria before billing: minimum 2 prior therapy lines (including immunomodulatory agent, proteasome inhibitor, and anti-CD38 monoclonal antibody), ECOG 0-2, adequate organ function, NO CNS involvement or pathology, NO active infection/GVHD/inflammatory disorders, and NO prior CAR-T therapy. (3) Providers must document all exclusion criteria verification in medical record. (4) Claims submitted without precertification approval or incomplete eligibility documentation will be denied. (5) Ensure billing team understands Q2055 is the only covered HCPCS code; all other indications (solid tumors, etc.) are experimental and not covered. Do not submit claims for non-approved indications.
