Front desk staff must verify patient diagnosis is limited to BCG-unresponsive NMIBC with carcinoma in situ (ICD-10 C67.0-C67.9 and D09.0) before scheduling. Add education for providers on contraindications (bladder perforation, gemcitabine hypersensitivity) to prevent claim denials. Claims submitted without precertification or exceeding 14 doses will be denied.
