Summary

Update billing software to flag HCPCS code J9183 for mandatory prior authorization before claims submission. Providers must contact Aetna precertification at (866) 752-7021 or fax (888) 267-3277 and submit Statement of Medical Necessity (SMN) forms for all cases. Ensure billing software enforces the 14-dose maximum limit over the 24-month treatment window (8 doses every 3 weeks for 6 months, then 6 doses every 12 weeks for 18 months). Front desk staff must verify patient diagnosis is limited to BCG-unresponsive NMIBC with carcinoma in situ (ICD-10 C67.0-C67.9 and D09.0) before scheduling. Add education for providers on contraindications (bladder perforation, gemcitabine hypersensitivity) to prevent claim denials. Claims submitted without precertification or exceeding 14 doses will be denied.
