Summary
Effective December 1, 2026, Anthem Blue Cross and Blue Shield Georgia is adding compression devices for lymphedema (HCPCS code E0659) to its precertification/prior authorization requirements for commercial plans. All claims for this service will require prior authorization approval before processing to avoid denials.
Action Required
By November 15, 2026: Billing team must update all billing system rules and prior authorization workflows to require precertification for HCPCS code E0659 (compression devices for lymphedema) for all Anthem Georgia commercial plan members. Front office staff must be instructed to verify prior authorization has been obtained before submitting claims. Update encounter forms and EMR templates to flag this code for prior auth review. Providers should submit prior authorization requests through Availity Essentials (https://Availity.com) or contact Anthem Provider Services using the member ID card number. Claims submitted without prior authorization approval will be denied for non-compliance with authorization requirements.
