Reimbursement Policies

Before August 1, 2026: Billing team must obtain and review the full revision history and detailed policy updates from EmblemHealth for each of the 13 updated LBM policies and Coding Edits Policy. Identify specific billing codes, coverage changes, and documentation requirements that differ from current practices. Update billing software rules, encounter forms, and clinical documentation templates accordingly for all affected services (ambulatory surgical procedures, preventive care, and laboratory testing for myocardial infarction, chronic heart failure, colorectal cancer, coronavirus, diabetes, gamma-glutamyl transferase, inflammation, microorganism identification, oral cancer, pathogen panels, prenatal screening, traumatic brain injury, alpha-1 antitrypsin, and urine culture). Communicate changes to providers and front-desk staff. Test billing software with new rules before effective date. Failure to implement changes may result in claim denials across commercial, Medicare, and Medicaid plans.
