Summary
Aetna has clarified coverage for breast ductal lavage and fiberoptic ductoscopy procedures. Only specific medically necessary uses are covered: ductal lavage as an alternative to aspiration for non-lactational nipple discharge with low cellular yield, fiberoptic ductoscopy with cytology for intra-ductal lesions, and ductoscopy as a guide for known intra-ductal cancer resection. All screening, risk assessment, and other investigational uses are not covered. Billing teams must update denial protocols to reject claims for non-covered indications.

By August 5, 2026: Billing team must update claim denial protocols to reject breast ductal lavage and fiberoptic ductoscopy claims submitted for non-covered indications including: screening (Z12.39), risk assessment, family history evaluation (Z80.3), personal history evaluation (Z85.3), inflammatory conditions (N61.0-N61.1), breast lumps (N63.0-N63.42), and other breast disorders (N64.3-N64.9, T85.44xA-T85.44xS). Claims are ONLY covered when submitted with ICD-10 diagnosis codes N64.52 (non-lactational nipple discharge), confirmed malignancy (C50.011-C50.A2), intraductal carcinoma in situ (D05.00-D05.12), or benign breast neoplasm (D24.1-D24.9) AND procedural notes document one of the three covered indications. Update billing software rules to auto-deny claims with non-covered ICD-10 codes. Train billing staff and providers on covered versus experimental indications. Create claim review checklist for billing team. Failure to implement will result in unnecessary claim denials and appeals.
