Summary
Beginning December 31, 2025, providers of Behavioral Support Services under Indiana Medicaid HCBS waivers must use new state-provided templates for all behavioral support plans, functional behavioral assessments, and quarterly reports. All existing plans and assessments must be updated to the new templates when next revised.
Action Required
By December 31, 2025: Providers offering Behavioral Support Services must download and begin using new state-provided templates from the BDS Portal Resources tab for all behavioral support plans, functional behavioral assessments, and quarterly reports.
