(4) Train billing and appeals staff on the prohibition against balance billing members for any overpayments identified during reviews. (5) Prepare appeals procedures and timeline for disputing recoupment decisions. (6) Monitor recoupment requests and implement procedures to appeal disputed amounts before recoupment from future payments occurs. Failure to provide adequate documentation will result in claim denials and recoupments; failure to comply with balance billing prohibition may result in regulatory penalties.
