Summary
Aetna's updated policy (effective 2026-03-18) clarifies that only standard mechanical peak flow meters and spacers are covered as DME for patients with asthma, bronchitis, emphysema, and obstructive pulmonary conditions. Electronic peak flow meters (PeakLog, AirWatch, etc.) are now classified as experimental/unproven and are not covered. Billing teams must ensure claims for electronic devices are denied and only mechanical meters are billed.
Action Required
By March 18, 2026: Billing team must update billing system to flag and deny all claims for electronic peak flow meters (product codes PeakLog Peak Flow Chronologger, AirWatch Peak Flow Monitoring System, VMX Mini-Log, EDC-spirometer, and any other electronic variants) as non-covered/experimental. Update claim scrubbing rules to ACCEPT only HCPCS codes A4614 (mechanical hand-held peak flow meter) and A4627, S8100, S8101 (spacers/holding chambers) when billed with qualifying ICD-10 diagnoses (J20.0-J21.9, J40-J47.9, J67.0-J67.9). Reject or downcode claims for S8096, S8097 if they include electronic peak flow meter components. Update prior authorization protocols to deny requests for electronic meters. Notify providers and DME vendors of this change to prevent claim denials. Educate front desk and prior auth staff that electronic peak flow meters require explicit member communication that these devices are not covered.
