Summary
Aetna has designated oral screening and lesion identification systems as experimental and investigational, effective immediately. Multiple oral cancer detection technologies, biomarker tests, and AI-based screening tools are now non-covered, including ViziLite, chemiluminescence, optical fluorescence imaging, HPV oral testing, and salivary biomarker tests. Billing teams must deny or reject claims for these procedures and update systems to reflect non-coverage status.
Action Required
Effective immediately (December 2, 2025): Billing team must update system settings to deny or non-cover all claims for oral screening and lesion identification systems. Specifically: (1) Add hard-stop edits in billing software for CPT codes 0429U, 82397, 87623, 87624, 87625, 87626, 40899, 41599, 41899; HCPCS codes D0431, G0476; and CPT codes 86663, 86664, 86665 when billed for oral cancer screening (ICD-10 Z12.81, C00.0-C10.9, D00.00-D00.08, D37.01-D37.09). (2) Update prior authorization protocols to automatically deny requests for experimental oral cancer screening technologies. (3) Configure claim rejection messages to reference CPB 0760 and indicate experimental/investigational status. (4) Notify providers in dentistry and oral-maxillofacial-surgery of non-coverage via staff communication and update encounter templates to prevent inappropriate billing. (5) Train front-desk and billing staff to inform patients these services are non-covered when scheduling. Claims submitted without these updates will be denied by Aetna with no recourse for provider appeal on experimental status grounds.
