Summary

Aetna's updated back pain noninvasive treatment policy (CPB 0232, effective 2026-04-23) clarifies coverage for physical therapy and back school while explicitly classifying quantitative muscle testing devices (MedX, Cybex, Biodex, Isostation B-200, Kin-Com) and multiple alternative therapies (cupping, kinesiotaping, auricular acupressure, sustained acoustic medicine, and others) as experimental/investigational and NON-COVERED. Billing teams must immediately stop attempting to bill these excluded services and update denial management accordingly.

Action Required

Action needed
By 2026-04-23: (1) Billing team must update claims scrubbing rules in billing software to AUTO-DENY any claims for quantitative muscle testing devices (MedX lumbar/cervical extension, Cybex Back System, Biodex System 3, Isostation B-200, Kin-Com, JTECH Tracker Freedom). (2) Physical therapy and orthopedic providers must immediately stop ordering/utilizing these devices for muscle testing; update clinical protocols and equipment lists. (3) Update denial reason codes to reference CPB 0232 for all rejected claims related to: cupping therapy, kinesiotaping, auricular acupressure, sustained acoustic medicine, high-frequency impulse therapy, matrix rhythm therapy, intermittent vertical lumbar traction, and Dr. Ho's 2-in-1 Decompression Belt. (4) Billing team must identify any in-flight claims (submitted within last 30 days) using CPT 97750 for muscle testing with these devices and either withdraw/resubmit with corrected coding or prepare for expected denials. (5) Front-office/intake staff must update patient education materials to reflect that these services are not covered by Aetna. (6) Revenue cycle leadership must audit accounts receivable for any payments received for these services in 2024-2026 and prepare for potential claim recoupment. Failure to implement these changes will result in claim denials, patient balance disputes, and compliance violations.
