Summary
Independence Blue Cross MA11.105l policy was updated effective 08/10/2026 to expand coverage terminology and add two new experimental/investigational procedures: Minimally Invasive Nasal Trabeculostomy (MIMS) and Minimally Invasive Nasal Trabeculostomy (MINT), along with new CPT code 1012T marked as experimental/investigational. Action Required
By 08/10/2026: Billing team must update billing system and claims processing rules to flag CPT code 1012T as experimental/investigational for Medicare Advantage members. Add MIMS and MINT procedure descriptions to encounter forms and claim submission templates. Update prior authorization workflows to deny coverage for 1012T unless member is enrolled in approved clinical trial. Communicate to ophthalmology providers that these procedures are not covered under standard Medicare Advantage benefits. Ensure claims submitted with 1012T on or after 08/10/2026 are rejected or sent for medical review with notification to providers that coverage is not available outside clinical trials.
