Summary
Independence Blue Cross updated policy 08.00.57w to add stricter criteria for sympathetic nerve blocks in CRPS treatment. Effective 08/10/2026, providers must now demonstrate sustained benefit from prior blocks—including 80% pain reduction, 2°C temperature increase, decreased medication use, and improved function—before additional blocks beyond the initial three are approved. This significantly tightens coverage requirements and prior authorization documentation.
Action Required
By 08/10/2026: Billing and prior authorization teams must implement the new CRPS sympathetic block approval requirements. (1) Update prior auth workflow to require documentation of failed first-line treatments (NSAIDs, corticosteroids, opioids, antiepileptics, antidepressants, muscle relaxants, physical therapy, or occupational therapy) for at least 2 weeks before initial stellate ganglion block (CPT 64510) or lumbar sympathetic block (CPT 64520) is approved. (2) For approvals beyond 3 blocks: Mandate provider documentation of ALL objective criteria—80% pain reduction, 2°C or greater temperature increase in ipsilateral limb from baseline, decreased pain medication use, improved functional measures (ROM/strength/ADL use), and decreased allodynia—before authorizing additional blocks up to maximum 6 per 12-month period at frequency not exceeding 2 per week. (3) Require providers to confirm comprehensive pain management program involvement (physical therapy, patient education, psychosocial support, oral medication) for all sympathetic block authorizations. (4) Update encounter templates and prior auth forms to include these objective measurement fields. (5) Train providers and clinical staff on new documentation requirements. Failure to obtain prior authorization under these stricter criteria or missing required objective documentation will result in claim denials for CPT codes 64510 and 64520, and for epidural infusion codes (62324) and related HCPCS codes (J0475, J0476, J2278, J2270, J2274). Communicate deadline to all contracted pain management and anesthesiology providers.
