Before February 16, 2026: Physical therapists submitting new Texas Medicaid enrollment applications must download and complete the Physical Therapy (PT) Attestation Form Regarding Location of Services from tmhp.com Provider Enrollment forms section. Attach completed form to all enrollment applications submitted on or after the effective date. Contact TMHP at 800-925-9126 for questions. Applications submitted before February 16, 2026 are not affected.
