Recall: Sodium Bicarbonate

Immediately: (1) Billing team must obtain the complete list of 49 recalled lot numbers from Exela Pharma Sciences LLC and the UPMC Health Plan. (2) Update billing system to flag or reject claims containing HCPCS code J7131 (Sodium Bicarbonate Injection) for any lot numbers on the recall list. (3) Notify all clinical staff, pharmacy, and providers to stop dispensing affected lots immediately. (4) Review all claims submitted in the past 30 days for J7131 with affected lot numbers and contact payers for guidance on claim adjustments or reversals. (5) Coordinate with pharmacy to verify inventory against recall list and quarantine any affected vials. Failure to implement these controls could result in billing for recalled/unsafe medication, claim denials on discovery, and potential patient safety liability.
