Summary
CMS requires all providers serving Medicare Special Needs Plan (SNP) members to complete annual Model of Care (MOC) training. UPMC Health Plan has developed an online training module accessible at upmchp.us/snpmoc that must be completed and attested to by February 29, 2024. This is a mandatory compliance requirement for all SNP providers.
Action Required
By February 29, 2024: All providers participating in UPMC for Life Complete Care (HMO-SNP) must complete the MOC training module. Actions required: (1) Providers or designated staff members must access upmchp.us/snpmoc using their tax identification number; (2) Complete the online training module; (3) Attest to completion within the system. Billing/administrative team should track completion status for all participating providers to ensure compliance.
