Summary
UPM Health Plan has released an updated version of the Notice of Medicare Non-Coverage (NOMNC) form effective January 9, 2025. All printed copies of the previous NOMNC form must be discontinued and replaced with the current version available on their provider website.
Action Required
By January 9, 2025: Billing team and front desk staff must immediately discontinue use of all printed NOMNC forms and replace them with the updated version from UPM Health Plan's website (https://www.upmchealthplan.com/providers/medical/announcements-updates/announcements/2025-announcements/2025-01-an-updated-version-of-the-nomnc). Update all encounter locations, provider offices, and patient-facing areas where the form is distributed. Failure to use the current NOMNC form may result in improper patient notifications and potential compliance issues with Medicare non-coverage documentation requirements.
