Summary
UPMC Health Plan has permanently closed their HIPAA Fax Line (412-454-7829) as of February 1, 2025. Members and providers must now submit Personal Representative Designation (PRD) forms and HIPAA documents via the Member Site digital portal or by mailing to the provided Pittsburgh address. Paper submissions must include member name, DOB, and/or Member ID on each page to avoid processing delays.
Action Required
By February 1, 2025: Billing team and front desk staff must immediately stop using fax line 412-454-7829 for any HIPAA document submissions to UPMC Health Plan. Update internal contact lists and provider communication materials to remove this fax number. For members needing to submit PRD forms or other HIPAA documents: (1) Direct members to use the Member Site (Digital Account) for fastest processing, or (2) If paper submission is required, mail directly to UPMC Health Plan ATTN: Medicare Enrollment, PO Box 2965, Pittsburgh PA 15230-2965. Ensure all paper submissions include member name, DOB, and/or Member ID on every page. Staff should be aware that faxed submissions will no longer be received and may be lost. Update any internal workflows, templates, or standing instructions that reference the closed fax line.
