Summary
Nostrum Laboratories has initiated a voluntary nationwide recall of all lots of Sucralfate Tablets USP 1 gram manufactured after June 2023 due to company bankruptcy and closure. The recall affects only Sucralfate products from Nostrum Labs; patients should contact their doctor or pharmacy to determine if their medication is affected. As of July 11, 2025, no adverse events have been reported.
Action Required
Immediately: Billing and clinical teams must identify all patients currently prescribed Sucralfate Tablets USP 1 gram manufactured by Nostrum Labs (manufactured after June 2023). Providers should contact affected patients to confirm medication source and discuss alternatives with them before discontinuing treatment. Front desk and billing staff should flag any claims related to recalled Nostrum Labs Sucralfate to ensure accurate documentation. Check pharmacy records and patient medication lists in EMR/billing system. Coordinate with pharmacy to ensure alternative Sucralfate products from other manufacturers are available for continuity of care. No billing code changes are required; this is primarily a patient safety and clinical coordination issue.
