Summary

Trividia Health has issued a nationwide voluntary recall of all True Metrix Blood Glucose Systems due to a software design flaw where the E-5 Error Code displays identically for two different conditions: critically high blood sugar (>600 mg/dL) or test strip errors. This ambiguity can delay or lead to improper treatment. As of January 16, 2026, 114 serious injuries and one death have been reported. Billing teams must alert providers and patients to this recall and ensure proper documentation of any related patient safety incidents or claim adjustments.
