Summary
CMS is launching the Medicare GLP-1 Bridge demonstration program effective July 1, 2026, to provide GLP-1 medications for weight reduction to eligible Medicare Part D beneficiaries who don't qualify through their Part D plan and don't have type 2 diabetes, moderate-to-severe sleep apnea, or MASH. This program operates outside Part D with centralized prior authorization and claims processing through a dedicated processor.

Establish process to identify eligible Medicare Part D beneficiaries (those without Part D GLP-1 coverage, type 2 diabetes, moderate-to-severe sleep apnea, or MASH). Configure billing system to route GLP-1 Bridge claims to the designated central processor rather than standard Medicare channels. Train providers on exclusion criteria to avoid inappropriate prescribing. Update patient intake forms to capture relevant medical history (diabetes status, sleep apnea severity, MASH diagnosis) to determine program eligibility. Incorrect routing or billing claims for ineligible beneficiaries will result in claim denials and payment delays.
