Summary

New York State is requesting a five-year extension of its Section 1115(a) Medicaid Redesign Team (MRT) demonstration waiver, scheduled to expire March 31, 2027. A 30-day public comment period is open until August 21, 2026, with a second public hearing on August 11, 2026. The extension proposes to continue current Medicaid managed care initiatives, amend workforce and Health-Related Social Needs (HRSN) programs, and discontinue coverage for TANF adults.

Action Required

Action needed
By August 21, 2026: Billing team and clinical leadership should review the MRT waiver extension request details on the NYS Department of Health website and consider submitting written comments if the proposed changes impact your practice. By August 10, 2026 at 4 p.m.: If your practice wishes to provide verbal comment at the second public hearing (August 11, 2026), register via the online form, include 'SP' before your name, and email 1115waivers@health.ny.gov to confirm registration. Monitor the MRT Listserv for updates on the final determination of the waiver extension, as approval or denial will affect NYS Medicaid coverage and reimbursement effective April 1, 2027. Update internal Medicaid eligibility and billing procedures once the waiver decision is finalized to account for discontinuation of TANF adult coverage (if extension is approved as proposed) and any amendments to HRSN service coverage.
