Summary
CalViva Health is discontinuing three Community Supports services effective January 1, 2027: Assisted Living Facility Transitions, Personal Care and Homemaker Services, and Short-Term Post-Hospitalization Housing. Billing teams must identify affected members, ensure services end by December 31, 2026, and coordinate transition planning to avoid service gaps and claim denials.

By December 31, 2026: Billing team must (1) run a report in the billing system to identify all active members currently receiving Assisted Living Facility Transitions, Personal Care and Homemaker Services, or Short-Term Post-Hospitalization Housing; (2) flag these members in the system to prevent authorization extensions beyond December 31, 2026; (3) notify providers and care management teams to initiate transition planning for affected members; (4) ensure no claims are submitted for these services on or after January 1, 2027, as they will be denied. Providers must complete all authorized services through each member's current authorization period, not extending past December 31, 2026.
