(2) Update prior auth requirements for extremity imaging—remove requirement for repeat x-ray in osteomyelitis/avascular necrosis management, remove conservative management prerequisite for TFCC instability imaging, and add meniscal tear/repair imaging allowances per updated MSK guidelines. (3) Revise spine imaging authorization to condense content and align frequency limits with MSK Interventional Pain Management guidelines. (4) Remove prior auth for carotid imaging in syncope cases and update vascular ultrasound indications to include giant cell arteritis diagnosis, asymptomatic carotid screening, traumatic vascular injury, and subclavian steal. (5) Update cardiac imaging criteria to allow repeat CAC imaging at 5-10 years when baseline score is zero, expand cardiac MRI allowances for hypertrophic/non-compaction cardiomyopathy, and clarify PET perfusion imaging language. (6) Provider education and billing software updates required. Claims submitted after September 19, 2026, not meeting updated criteria will be denied or require resubmission.
