Summary
Primary care physicians must now document all Initial Health Appointment (IHA) outreach attempts in Cozeva®, including unsuccessful contact attempts, to meet DHCS audit requirements. This documentation is mandatory for eligible Medi-Cal patients in Imperial County to demonstrate audit readiness.
Action Required
Immediately: Primary care providers must begin recording ALL IHA outreach attempts in Cozeva®, including unsuccessful contact attempts and completed IHAs for eligible Medi-Cal patients. Billing team should: (1) Audit current Cozeva® documentation practices to ensure outreach attempts are being captured; (2) Train front desk and clinical staff on proper documentation of both successful and unsuccessful outreach contacts in Cozeva®; (3) Implement workflow to ensure every outreach attempt is logged before marking a patient as unable to contact; (4) Review the 'Tips for Completing the Initial Health Appointment' resource in the Provider Library (Medi-Cal > Education, Training and Other Resources section) for current IHA requirements, service codes, and documentation guidance.
