Summary
Effective September 1, 2026, Anthem Blue Cross (Medi-Cal) will enforce stricter claims editing to ensure all services billed for inpatients use Place of Service (POS) 21 (Inpatient Hospital) or higher, regardless of where the face-to-face encounter occurred. Claims submitted with incorrect POS codes will be rejected and require correction/resubmission before reimbursement.
Action Required
By August 31, 2026: Billing team must audit all current workflows and claims submission processes to identify where inpatient status is captured and reported. Update billing system to automatically assign POS 21 (Inpatient Hospital) or appropriate inpatient POS codes for all claims where the patient is registered as an inpatient, regardless of service location. Providers must verify patient inpatient status at time of service and ensure encounter documentation reflects inpatient admission status. Update claim submission templates and billing software validation rules to reject any non-inpatient POS codes for patients with active inpatient registrations. Train billing and front-desk staff on identifying inpatient status during registration and check-in. Implement a verification step in the claims submission workflow to cross-check patient status against POS code before transmission. Failure to comply will result in claim denials and mandatory resubmission, creating rework and payment delays.
