Summary

Anthem Blue Cross (California) now requires all in-network, participating care providers to annually attest that patients have access to member grievance and appeals forms and filing instructions on-site. Providers must complete a Consumer Grievance and Appeals Attestation Survey via Medallia to confirm compliance with DMHC and California Health and Safety Code regulations. Anthem will contact organizations that have not confirmed adherence.

Action Required

Action needed
By August 1, 2026 (or upon receiving notice from Anthem): Ensure grievance and appeals forms and filing instructions are physically available at patient-facing facilities. Complete the Consumer Grievance and Appeals Attestation Survey on Medallia (medallia.com) to attest compliance. Front office or administrative staff should confirm materials are accessible. If you receive direct notice from Anthem, respond promptly to gaattestationsurvey@anthem.com. Failure to attest may result in escalated communication from Anthem and potential compliance issues with DMHC regulations.
