Summary
CareSource Ohio Medicaid is updating pharmacy policies for 14 specialty medications effective November 1, 2026. By October 15, 2026: Billing and pharmacy teams must access each revised and new policy document on CareSource.com (Providers > Tools & Resources > Provider Policies > select Ohio Medicaid > Pharmacy) to identify: (1) prior authorization requirements for the 8 revised/new medications, (2) any updated coverage criteria or clinical guidelines, and (3) billing/reimbursement changes. Update billing system rules, prior auth workflows, and clinical staff alerts accordingly. Failure to implement required prior authorizations will result in claim denials for affected medications (AVLAYAH, ELAPRASE, KANUMA, LEQVIO, LOARGYS, NATALIZUMAB, VYVGART, YARTEMLEA). Ensure providers are notified of any new documentation requirements before the November 1 effective date.
