Summary
Blue Cross Blue Shield of Kansas is updating its Coverage Exception (CE) process effective September 1, 2026. The key change requires members to demonstrate inadequate response to or intolerance of BOTH two generic formulary medications AND two brand-name formulary medications before a non-formulary medication can be approved. Existing approved CEs will continue through their current authorization period, but renewals will use the new criteria.

By August 31, 2026: Billing and clinical teams must update internal workflows and prior authorization submission templates to reflect the new CE criteria requiring evidence of failure on two generic AND two brand-name formulary medications. Update provider education materials and any automated systems that process CE requests to ensure compliance with the new dual-medication requirement. For members with existing approved CEs, continue processing claims under current authorizations through their expiration date. Beginning September 1, 2026, all new CE requests and renewal requests must document the four-medication trial requirement or claims will be denied. Coordinate with providers to ensure adequate medication trial documentation is captured in patient records before requesting coverage exceptions.
