REQUIREMENTS: 1. By June 30, 2026: Billing team and clinical staff must verify all provider directory information in Presbyterian's provider portal (physical health providers) and behavioral health portal (behavioral health providers). This is a federal No Surprises Act requirement with no exemptions. Contact providerdemo@phs.org or PHPTCBH@magellanhealth.com for assistance. Failure to verify may result in directory inaccuracies affecting patient access and claims routing. 2. Effective immediately: Implement process to ensure both eGFR and uACR tests are ordered and documented annually for all patients with diabetes. Update EHR templates to flag patients due for kidney health evaluations. Bill these tests when clinically indicated for KED HEDIS measure compliance. Use full quantitative urine tests (not dipsticks) for uACR testing. 3. Effective immediately: Correct medical record documentation deficiencies identified in 2025 audit: (1) Advance Directives—ensure ALL patients age 18+ have documented patient education, declination, or signed copy with date and method recorded; (2) Immunizations—list all immunizations with dates or document current status/declination for all members. 4. By June 30, 2026: Ensure at least one provider or office staff member from each contracted physical health group and behavioral health organization attends the required annual Provider Education Conference & Webinar Series training (sessions available June 10-11, 2026). Behavioral health providers must also attend one quarterly town hall (first one June 15, 2026). Track attendance and maintain proof of completion. 5. Effective immediately for Dual Plus and CISC providers: Complete required year-round on-demand trainings (Presbyterian Dual Plus Provider Training and Children in State Custody Extended Provider Network Training available on-demand). Providers must complete personally; office staff cannot substitute. Maintain attestation of completion.
