Summary
Mutual of Omaha released an August 2026 policy update with new medical policies (including Sacral Nerve Stimulation), updates to existing policies (Bone Graft Materials and Digital Therapeutics), and numerous Avalon LBM policy updates affecting laboratory and diagnostic testing. By October 30, 2026: Billing team must implement all material policy changes including: (1) verify coverage status for new CPT/HCPCS codes listed in Avalon LBM updates (F2019, G-codes, M-codes, T2015) in billing software; (2) confirm prior authorization requirements for Sacral Nerve Stimulation (202604); (3) update Bone Graft Materials policy coverage (202504) to reflect new IE materials and removal of platelet rich plasma terms; (4) update Digital Therapeutics coverage (202505) to include newly covered applications (Drowzle Pro, Luminopia One, MindMotion GO, MyVisionTrack, Parallel, Regulora, RelieVRx). Front desk staff should route claims for newly listed services through prior authorization queue per updated prior auth lists. Obtain complete policy language from MedMutual.com/Provider > Policies and Standards > Medical Policies before October 30 to ensure compliance. Failure to update codes and coverage rules will result in claim denials or delayed reimbursement.
