Summary
Nevada Medicaid has updated 50+ prior authorization and request forms with cosmetic changes to reflect the transition to Nevada Medicaid branding. No content changes were made to the forms themselves. Providers must immediately begin using the 07/13/2026 dated versions; forms submitted on older versions will be rejected starting August 13, 2026.
Action Required
Immediately: Billing team and administrative staff must download and replace all Nevada Medicaid FA-series forms (FA-1 through FA-101, plus Dental PA Instructions) from the Nevada Medicaid Provider Portal with versions dated 07/13/2026. Update all form templates in billing software, EMR systems, and shared document repositories. By August 13, 2026: Ensure 100% compliance with new forms or all submissions on old versions will be returned for resubmission, causing claim processing delays and prior authorization denials. Assign staff to verify forms are current before submission and establish a reminder to check the Nevada Medicaid Provider Forms webpage monthly for future updates.
