Summary

Nevada Medicaid has updated the Provider Web Portal (PWP) and the FA-24 form to streamline Personal Care Services (PCS) initial prior authorization submissions. Effective immediately, providers can submit PCS Initial PA requests directly through PWP, and the updated FA-24 form (dated 07/21/2026) now consolidates both initial PA and provider transfer requests. The previous FA-24 version must be discontinued by September 21, 2026, after which requests on old forms will be rejected.

Immediately: Billing team and all providers must transition to the updated FA-24 form (dated 07/21/2026) for all PCS initial prior authorization and provider transfer submissions. Discontinue use of the previous version of Form FA-24 in all workflows and billing systems. By September 21, 2026: Ensure all staff are trained on submitting PCS Initial PA requests through the Provider Web Portal using the new portal feature. Update all internal templates, checklists, and documentation references to reflect the merged FA-24 form. After September 21, 2026: Claims or PA requests submitted on any previous version of FA-24 will not be accepted and will be rejected. Verify the updated form is available on the Nevada Medicaid Provider Forms webpage and update any stored or printed copies.
